Nominations are accepted until November 1 of each year, with nominations reviewed by the Mayor, one City Council Member, and staff. Carver Community Service Award winners will receive a plaque and be recognized in City publications for their achievement.
Completed forms should be dropped off at City Hall or e-mailed to [email protected] by November 1.
